慢性化したＰＴＳＤではトラウマ記憶が断片化しているため、ツラい体験の核心部分の記憶を回避するよう思い出さなくなっていたり、思い出しにくくなっています。

いつまでも回避し続けると、記憶に触れたときの嫌な感情（恐怖、絶望、不安）に慣れることができず、より一層回避が強くなるという悪循環が生まれます。

また、トラウマ記憶の全体を整理することができないことで実際よりも不安を感じたり恐怖を強く感じたり、必要以上に自分を責めてしまったり、人間不信になることもあります。

言い換えると、ＰＴＳＤのトラウマ記憶とは

昔の怖かった出来事の記憶ではなく、今でも恐怖を生み出し続けている特殊な記憶

ということになります。これを普通の記憶に戻すこと、つまり「トラウマ記憶は過去のことであり、思い出しても今の自分が被害を受けるわけではないこと」を実感してもらうことが情動処理の認知療法となります。長い間トラウマ記憶に苦しんでいるとその影響を受け、自分や周りの人々に対してネガティブな受け取り方や感じ方をする癖がついていることも多くあります。認知の歪み、それを修正していくことも治療の目標になります。

虐待のように長期的に繰り返される被害を受けた場合に特にこのことが重要になります。

治療のためにトラウマについて話しをしながら気持ちや感情を整理していく認知行動療法を行います。その代表的な心理療法が持続エクスポージャー法で、スモールステップで安心、安全を確かめながら心理士と一緒に決まった手順に従い記憶を想起し、今までとは違った捉え方や考え方感じ方を学んでいきます。
